• The dental membership program is NOT an insurance. We can only offer this benefit to patients that are not currently covered under a dental health plan.

• Members of the program must receive care from Village Family Dental Associates. This is NOT transferable to other dental offices or specialties.

• Membership is for One year, beginning the day of enrollment.

• Enrollment can start any date you choose.

• Membership dues are payable in full upon enrollment and are Non-refundable.

• Cash, checks or credit card payments are accepted.

• It is the member's responsibility to schedule all appointments offered in this program.

• 48 hour notice is requested if you must reschedule your appointment.

• Payments for “additional dental services” are due on the day of service to receive the 15% or 13% discount. 15% discount by paying with cash or check. 13% by credit card.

Is this an Insurance Plan?

No, Village Family Dental is not an insurance company. This is simply a dental membership plan for those without dental insurance.

Can I sign up my entire family?

Yes. Each patient is individual and can join anytime. You can have different renewal dates for every member of your family if you wish.

What happens if I obtain Dental Insurance while I'm a member of the membership?

Our membership program is only for uninsured. If you become insured, your membership will be terminated and dues are non-refundable.

Can my FLEX plan be used for the annual fee for reimbursement?

Most of the flex plans do reimburse for dental procedures. However, it is up to you to contact your administrator of your flex plan to know for sure.

I have Dental Insurance, can I join your program?

You are not eligible for this membership if you are covered under a dental insurance program of any kind.
